Fixed Food Service License

A Fixed Food Service License is required for a fixed or mobile restaurant, cafeteria, cafe, bar, rental hall, catering kitchen or similar place where food or drink is prepared for consumption on the premises or elsewhere, and any other eating or drinking establishment or operation where food is served or provided for the public.

To begin your application for a Fixed Food Service License, you will need to submit a completed plan review, and the required fee. After submitting your completed plan review, a specialist will review your paperwork within 30 days. More information on the plan review process can be found here.

Once our specialists approve your plan review, you will pay a license fee and schedule a pre-opening inspection with the plan review specialist. 

​

We understand that you may consider contacting a food consultant. They will complete your paperwork and make sure all documentation is turned in at the time of submission. Missing documents can delay the review process. Please be sure to go over the checklist and submit all the paperwork at the time you submit the packet.

  3. In Person: Visit our Environmental Health Division at the address above

Payment must be included for your plan review to be processed.
